Findings: Witness #2 (W2) has a history of aggressive behavior towards the Alleged Victim (AV). W1 was moved to a different pod than AV to avoid altercations between them. On or about March 25, 2024, W1 banged on the door of his/her pod, and when the door opened, staff allowed W2 to leave his/her pod and enter the common area, which gave W2 access to where AV's room is. Staff heard AV yelling and entered his/her room and found W2 bending AV's toes back. W2 had also pulled AV out of bed by his/her hair and hit AV. AV suffered a red mark to his/her face. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
