Findings: Alleged Victim (AV) is known to sustain falls. There is conflicting information regarding how many safety checks staff are required to complete. In the service plan and TSP, four (4) safety checks every shift is written but in the incident report it states the facility is doing eight (8) safety checks per shift. On or about March 25, 2024, AV was found on the ground with AV’s pants/depends below the waist, that were wet in the hallway. The facility failed to provide a safe environment for AV, which lead AV to sustain a fall, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: ALFCP25-00353 $375.00 fine assessed
