Findings: Based on facility documentation and interviews, while Alleged Victim (AV) was in the bathroom, he/she developed a nosebleed, and was in a great deal of facial pain from a fall that had occurred on March 20, 2024. On March 21, 2024, AV received pain medication and anti-anxiety medication outside the parameters prescribed by AV's physician. AV experienced no ill-effects from receiving the two medications outside prescribed parameters. The Facility failed to provide a safe medication administration system which is a violation of Oregon Administrative Rules.
