Findings: The facility failed to provide appropriate services according to the Alleged Victim (AV)'s needs. AV is service planned for assistance with transfers, toileting care, and assistance with showering cares. AV has a history of urinary tract infections, with one occurring on February 23, 202024. AV did not receive AV's showers from February 14, 2024 to March 22, 2024 with minimal interventions, put into place for missed showers. AV's skin was in contact frequently with urine, which contributed to AV's rash and pressure sore. The facilities only interventions were to apply a barrier cream to AV's back and a powder to AV's peri area. AV sustained rashes in AV's peri area, a pressure sore and a rash on AV's back that began to ooze pus that caused AV pain. .AV experienced significant skin injuries, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
