Findings: The facility failed to provide appropriate services according to the Alleged Victim (AV)'s needs. AV has a history of skin issues. AV is service planned for assistance with toileting cares. AV sustained a rash to AV's skin near the peri area. AV has been left in urine-soaked briefs, clothes and bedding for long periods of time. AV's service plan did not include information about AV's peri area rash or treatment. AV experienced prolonged rash, skin injuries, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
