Findings: Alleged Victim (AV) relies on the facility for his/her care. AV and Witness 1 (W1) have a history of behaviors and resident-to-resident altercations. AV is care planned for the shared bathroom door to be locked to keep W1 from entering. According to an investigation, on or about March 9, 2024, W1 entered the shared bathroom where he/she hit AV on both sides of his/her face, resulting in pain and redness. The facility failed to follow the care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
