- Findings
- Alleged Victim (AV) has a known habit of standing over other residents when in the dining room . Witness 1 (W1) has a known history of being agitated when other residents pick up h/h items off the table, and when other residents are in h/h perceived space. On March 26, 2024, AV was wandering within the facility when s/he entered the dining room and approached W1’s table. AV picked up W1’s plate, W1 tries to take the food back from AV. AV and W1 begin swatting at each other back and forth. W1 grabs AV’s arm and does not let go until staff intervene. As a result of the altercation, AV experienced redness on h/h arm. AV and W1 have previously been involved in a resident-to-resident altercation with one another on February 12,2024. Interventions in place to mitigate W1’s behaviors include staff are to intervene and redirect if they see W1 becoming agitated. If another resident is in W1’s perceived space, to redirect other residents and if they approach W1 and attempt to touch h/h food. Interventions put into place to mitigate AV’s behaviors include staff to provide oversight when AV is in the dining room watching a movie, staff are to put a movie on in AV’s room if s/he is showing agitation, and redirect AV away from other residents until s/he calms down. The facility failed to implement appropriate person-centered interventions to address W1’s known triggers and behaviors towards other residents leading to a physical resident-to-resident altercation between AV and W1.
