Findings: The facility failed to provide appropriate services according to the Alleged Victim (AV)'s needs. AV has a history of rashes. AV is service planned for incontinence and peri care. AV sustained a rash to the back of AV's thighs. During investigation, it was determined AV being left in urine-soaked briefs contributed to AV sustaining a rash to the back of AV's thighs and genital area. The facility's only interventions were to apply creams and bandages to AV's rash with no other interventions set in place. AV's rash caused AV unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
