Findings: The Alleged Victim (AV) relies on the facility for their care and to ensure a safe environment. The AV and Witness 1 (W1) have known behaviors and a history of altercations with each other. According to an investigation, on or about, March 24, 2024, the AV’s walker collided with W1’s walker. W1 hit the AV in the left forearm multiple times causing bruising and unreasonable discomfort. The facility failed to ensure a safe environment for AV, which is a violation of resident rights, is considered neglect of care, and constitutes abuse.
Sanction: RCFCP25-00750 $250.00 fine assessed
