Findings: According to the documentation, the facility failed to provide a safe environment for the Alleged Victim (AV) by allowing them to leave the locked facility unsupervised. On or about March 30. 2024, the AV was able to leave the locked facility unsupervised. The AV made it to the parking lot where they were found approximately 20 minutes after they were last seen in the facility, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
