Findings: On or about April 2, 2024, the Alleged Victim (AV) suffered a fall in his/her room. AV didn't have complaints at the time of the fall but complained of pain in his/her left leg. AV moved into the facility on March 31, 2024, however, no documentation was completed for AV at the time, and no staff were aware of AV's care needs. Also, no move in assessment was completed for AV prior to AV's move in. The facility's failure to ensure the safety of AV is a violation of resident rights, is considered neglect of care and constitutes abuse.
