Findings: The Alleged Victim (AV) relies on the facility for his/her care and is care planned for toileting assistance. On or about, March 27, 2024, AV used his/her call pendant and was calling out for staff overnight without a response. AV was found soiled in his/her brief/bedding the next morning, resulting in unreasonable discomfort and lack of dignity. It was also discovered that around this time, it was known that the call system was not working properly. The facility failed to provide appropriate services, which is a violation of resident rights, is neglect of care which constitutes abuse. The allegation that Alleged Perpetrator 2 (AP2) and Alleged Perpetrator 3 (AP3) neglected AV was investigated and determined to be not substantiated.
