Findings: The Alleged Victim (AV) is a known fall risk and has suffered numerous falls at the facility, including one that resulted in a spinal fracture. AV returned to the facility in February 2024 after recovery from the spinal fracture. On or about March 8, 2024, AV suffered another fall where AV suffered a laceration to his/her hand and head, requiring stitches. The facility failed to update AV's care plan to ensure effective fall interventions were in place. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
