- Findings
- The facility failed to follow Witness 1's (W1) care plan to be redirected by staff and escorted to minimize his/her behaviors. The failure resulted in W1 approaching the Alleged Victim (AV) and splashing a cup full of water all over AV, causing AV emotional distress, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
