Findings: The Alleged Victim (AV) and Witness #1 (W1) have a history of altercations. AV and W1 entered into an altercation on March 2, 2024 where W1 hit AV in the head approximately 4 times. AV and W1 engaged in another altercation where W1 pulled AV's hair and grabbed AV's wheelchair. The facility failed to provide a safe environment, placing residents at risk for harm. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
