Findings: The Alleged Victim (AV) was a known fall risk and had suffered falls in previous months. On or about March 24, 2024, AV fell while showering and suffered a broken shoulder. AV was placed on frequent checks, however, AV fell again. Documentation shows that frequent checks were being completed for AV even as AV was out of the facility. The facility failed to properly care plan to ensure AV would be safe from falls. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
