Findings: Witness 1 (W1) service plan dated March 6, 2024, indicates W1 becomes distressed and will try to get involved if residents become agitated or engage in arguing/disagreements. Staff to provide W1 with reassurance and attempt to redirect W1 if W1 becomes distressed. W1 leaves the door to the apartment opened during the day. On or about March 27, 2024, W1 and Alleged Victim (AV) switched rooms. On or about April 3, 2024, W1 was in h/h room when AV came into W1's room thinking the room was AV's. AV was irritated because AV thought W1 was taking AV's things. W1 is upset and frustrated with AV coming into W1's room. This resulted in a resident-to-resident altercation, AV reports W1 punched AV, AV appeared in distress. The facility failed to appropriately care plan and implement reasonable interventions to address AV wondering into W1 room, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
