Findings: The facility failed to provide appropriate services according to the Alleged Victim's (AV) care needs. On April 5, 2024, AV was discovered not wearing an adult brief, soaked in urine with feces smeared on both his/her person and bedding. Based on facility documentation and interviews, the facility failed to provide appropriate services for AV's toileting and incontinence care need between March 14, 2024, and April 3, 2024. AV was reported to have increased incontinence and increased confusion during this time. The facility's failure resulted in AV experiencing unreasonable discomfort and a loss of personal dignity by being dirty and unkept, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
