Findings: The Alleged Victim (AV) relies on the facility for their care and to ensure a safe environment. The AV and Witness 1 (W1) have known behaviors and a history of altercations, and are care planned to be redirected from one another. According to an investigation, on or about, April 5, 2024, W1 patted the AV on the head, the AV struck W1, who then proceeded to hit the AV in the face and on the arm. The facility failed to follow the care plan and implement interventions for the AV, which is a violation of resident rights, is considered neglect of care, and constitutes abuse.
