Findings: The Alleged Victim (AV) relies on the facility for care and is service planned for total assistance with grooming, shaving, nail care, and bathing. According to an investigation, on or about April 9, 2024, AV was observed with wax build-up inside both ears, built-up debris on the scalp and behind the ears, overgrown finger and toenails, and was malodorous, resulting in a loss of personal dignity. The facility failed to follow AV’s care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP24-00739 $500.00 fine assessed
