- Findings
- Alleged Victim (AV) is independent in transferring and ambulation and uses a walker to ambulate. AV has history of falls on or about February 02, 2024, February 26, 2024, March 31, 2024. On or about April 09, 2024 at approximately 12:00am, AV was found on the floor in AV's bathroom between the sink and toilet. AV was rubbing AV's, left leg and was saying "Ow" . AV was assessed and transferred back to bed with a determination of no injuries found. At approximately 5:00 a.m. AV was complaining of pain and could not bear any weight on AV's leg. EMS was called and AV was transported to the hospital for further evaluation and diagnosed with a hip fracture with the need for surgical repair. Interventions able to be recalled reducing AV’s risk of falls included safety checks every hour, toileting assistance, and making sure AV's walker is next to AV Although AV's service plan states AV is independent with ambulation, The incident reports for AV’s falls state, “Staff and family are aware that AV can be unbalanced” The facility failed to appropriately implement interventions and service plan for AV ongoing falls and around AV's balance issues to reduce or eliminate fall risks, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
