Findings: On or about March 12, 2024, Witness #1 (W1) and the Alleged Victim (AV) engaged in an altercation. W1 and AV have had previous altercations. Neither resident suffered any injury during the altercation, however, failure to ensure interventions were enacted placed both residents at risk for harm. The facility's failure to ensure residents safety is a violation of resident rights, is considered neglect of care and constitutes abuse.
