Findings: The facility failed to appropriately monitor Witness 1 (W1) according to his/her known behavior and prior altercations. On or about March 17, 2024, W1 struck Alleged Victim (AV) on top of his/her head. Based on facility documentation and interviews, AV got into W1's personal space when AV picked items off of the ground near W1. AV triggered W1 which resulted in a physical altercation. The facility's failure to identify W1's potential trigger and redirect W1 prior to W1 becoming agitated resulted in a physical altercation causing unreasonable discomfort to AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
