Findings: Alleged Victim (AV) is independent in ambulation and transfers. AV has a history of injury and non-injury falls. Following AV's injury fall on January 5, 2024, no new fall prevention interventions were implemented by the facility. On March 18, 2024, staff were alerted by another resident that AV was on the floor; staff found AV sitting on h/h bottom, leaning against h/h bed, with AV reporting that s/he had fallen while attempting to take h/h self to the bathroom. On March 22, 2024, AV was found to have bruising on the left side of h/h back. The bruising found on March 22, 2024, matches contact the AV's back made with the bed or ground with the fall on March 18, 2024. The facility failed to appropriately care plan and implement reasonable interventions to address AV’s increasing and ongoing falls, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
