Findings: Alleged Victim (AV) is known to have a history of falls. Between February 10, 2024, through March 6, 2024, AV sustained 13 falls, with multiple falls in one day on three (3) occasions. On or about March 6, 2024, AV had an unwitnessed fall in the common area which resulted in a broken collar bone and pain to AV. The temporary care plan did not include new fall interventions that were put into place for AV. The facility failed to care plan and mitigate AV’s risk of falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP24-01077 $1500.00 fine assessed
