Findings: On or about APril 12, 2024 • Alleged Perpetrator 2 (AP2) administered AV medication that belonged to another resident causing unreasonable discomfort resulting in AV being transported to the hospital for evaluation and treatment. AP2’s actions are a violation of resident rights, are considered neglect of care and constitutes abuse. The facility failed to administer medication as ordered which is a violation of Oregon Administrative Rules.
