- Findings
- Alleged Victim (AV) uses a wheelchair for ambulation, is unable to bear any weight, and relies on multiple-person assistance with all transfers. AV has a history of falls, and behaviors. Between March, 1, 2024 and April 4, 2024, AV was documented to have been found on the floor or to have "placed h/h self on the floor" 44 times, with several resulting in injury. AV required one-to-one staff supervision during NOC shift on April 15, 2024 due to AV placing h/h self on the floor as soon as staff would leave the room. The facility did not have sufficient staffing to continue AV's one-to-one supervision. AV was on Significant Change of Condition Monitoring, but the monitoring only listed AV's regular falls/found on the floor and did not suggest or implement any interventions. There are no Interim Service Plans with behavior or fall prevention interventions documented afterMarch, 28, 2024, which asked staff to ensure that AV had stuffed animals in bed with h/h. The facility failed to implement reasonable behavior or fall prevention interventions, which resulted in AV continuing to be found on the floor after April 4, 2024.
