Findings: According to the documentation, the facility failed to properly care plan appropriate interventions to ensure the safety of the Alleged Victim (AV) who has a history of falling off the couch. On or about January 23, 2024, the AV suffered two falls off the couch while sleeping. The first fall happened at approximately 4:28 sustaining no injuries. Around 7:30 am on the same morning, the AV was found on the floor in front of the couch with a skin tear on their ear, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP24-00741 $1000.00 fine assessed
