Findings: According to the documentation, the facility failed to follow the care plan for the Alleged Victim’s (AV) known fall risk. According to the AV’s service plan, when the AV goes to bed, staff are to ensure bed is in the lowest setting, bed alarm is in place and on, and floor mat is next to the bed. On or about April 16, 2024, staff went into the AV's room and found the AV on the floor next to their bed. The bed was not in the lowest position, alarm was not in place and the fall mat was not next to the bed, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
