- Findings
- Alleged Victim (AV) and Witness 1 (W1) have limited cognition and memory. AV's Service plan: AV is independent with ambulation, AV uses the handrails at times while walking to and from h/h apartment, AV has balance issues. Interventions in place for AV: offer AV to join activities to provide additional oversight, , if staff see AV walking to h/h room they are to offer AV assistance to h/h room and sitting in a location of AV’s choosing. W1 Service plan: if staff see W1 start to get agitated with other residents they are to offer to take W1 to h/h room to see h/h cat, Staff are to intervene and redirect when they see W1 becoming agitated with another resident in W1’s perceived space. AV and W1 have a history of resident-to-resident altercations with one another, including an altercation on January 30, 2024. On April 17, 2024 AV attempted to open the door to W1's apartment; W1 closed the door and pushed AV away from the door, causing AV to fall to the ground. The facility did not document the implementation of any behavior interventions for either AV or W1 after the altercation on January 30, 2024. The facility failed to implement behavior interventions following AV and W1's resident-to-resident altercation on January 30, 2024, which resulted in an altercation on April 17, 2024 where W1 pushed AV to the ground in the hallway.
