Findings: According to the documentation, the facility failed to properly care plan appropriate interventions for the Alleged Victim’s (AV) known fall risk associated with alcohol consumption. On or about April 16, 2024, the AV called for help and was found on the floor by their wheelchair. The resident was sent to the ER for assessment and was found to have a hematoma on their forehead and a broken clavicle. There was no documented evidence of progressive interventions to ensure the AV’s safety when they consumed alcohol, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
