Findings: The Alleged Victim (AV) relies on the facility for his/her care. According to an investigation, on or about April 22, 2024, a facility staff member transferred AV, resulting in a skin tear on his/her leg. The facility failed to provide appropriate services for AV’s needs, including care planning and lack of instruction, which is a violation of resident rights, is neglect of care which constitutes abuse.
