Findings: According to the documentation, the facility failed to provide appropriate staffing to meet the scheduled and unscheduled needs of the residents. On or about April 6, 2024, the Alleged Victim (AV) suffered an unwitnessed fall resulting in a fractured clavicle. According to the Alleged Perpetrator 2 (AP2) and the facility report, the AP2 was the only care staff on shift at the time of the fall due to the other care staff calling out, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
