Findings: According to the documentation, the facility failed to follow the service plan for Witness 1 (W1) to provide 1:1 assistance for know behaviors. On or about April 21, 2024, the Alleged Victim (AV) was involved in an unwitnessed resident to resident altercation with W1. Staff heard a loud sound and found the AV on the ground, who then complained of head pain. W1 required 1:1 care according to the service plan which. Since the incident was unwitnessed, the service plan for W1 was not being followed, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
