Findings: According to the documentation, the facility failed to properly care plan appropriate interventions to prevent the Alleged Victim (AV) from experiencing numerous falls. From approximately April 01, 2024, through April 26, 2024, AV told staff they had fallen about 5 times and had to wait up to 45 minutes for staff to respond to their call light. There were no interventions placed on the AV’s service plan after the AV told facility staff they had fallen. The failure to implement appropriate fall interventions on the service plan to prevent future falls put the AV at risk of harm from future falls, which is a violation of resident rights is considered neglect of care and constitutes abuse.
