Findings: The Alleged Victim (AV) has a catheter and requires catheter care by the facility. On or about April 24, 2024, it was noted by AV's behavioral health specialist the AV's catheter was not catching AV's urine output as it is supposed to. Staff were having to clean and change AV multiple times per day due to urine leaking down AV's leg, into his/her socks. Staff knew that AV's catheter was leaking for at least 10 days, however, the facility failed to have home health or anyone come in to assist AV with ensuring the catheter was working properly. AV suffered unreasonable discomfort and a loss of personal dignity. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
