Findings: The Alleged Victim (AV) is known to be at risk for falls due to history of injury, unsteady gait, and weakness. Between February 2024 and April 2024, AV was found on the floor by his/her bed 6 times, 5 of those falls were within 3 weeks. During this time frame, no fall interventions were noted in AV's progress notes or incident reports. On or about April 11, 2024, AV was found on the floor of his/her room with cut on his/her lower left leg. AV was taken to urgent care where he/she received 9 stitches to his/her leg. Respondent failed to ensure proper care planning to reduce falls. This constitutes abuse by neglect, as outlined in OAR 411-020-0002(1)(b)(A)(i).
