Findings: The facility failed to provide appropriate services according to the Alleged Victim’s (AV) care needs. AV has a history of bruising and receiving skin tears on his/her arms and legs. Based on facility documentation, outside provider notes state to have AV wear long sleeves, pants and/or elastic stockings during awake hours as an intervention to prevent bruising and skin tears. On April 30, 2024, AV was observed wearing long sleeves but did not have pants or elastic stockings on his/her legs. AV also had a bruise on his/her left arm. The facility's failure to appropriately care plan AV’s care needs, caused AV unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
