Findings: The Alleged Victim (AV) receives his/her medication from facility staff. On or about April 27, 2024, AV was not given his/her medication as ordered. AV's missed medications were not properly recorded on the facility medication administration system. Alleged Perpetrator #2 (AP2) failed to administer or record AV's medication. AP2 was not properly trained prior to providing medication administration. The facility's failure to provide proper training and have a safe medication administration system is a violation of Oregon Administrative Rules.
