Findings: On or about April 27, 2024, the Alleged Victim (AV) did not receive his/her medications as ordered. Alleged Perpetrator #2 (AP2) failed to pass medications as ordered. AP2 was not trained properly prior to passing medication at the facility, resulting in AP2 not being able to find medication for AV. The facility's failure to properly train agency staff is a violation of Oregon Administrative Rules.
