Findings: On or about April 28, 2024, the Alleged Victim (AV) did not receive his/her blood pressure medication. AV's medication was not available at the facility and had to be re-ordered by staff. The facility has had multiple medication errors within the six months prior to this allegation, leading to a civil money penalty. The facility's failure to provide a safe medication administration system is a violation of Oregon Administrative Rules.
