Findings: On or about April 27, 2024 and April 28, 2024, the Alleged Victim (AV) did not receive two medications during the med pass. Alleged Perpetrator #2 (AP2) and Alleged Perpetrator #3 (AP3) were not able to find the medication for AV to be able to pass the medications. AV's medications were available but couldn't be found by AP2 and AP3. The facility's failure to provide a safe medication administration system is a violation of Oregon Administrative Rules.
