Findings: The Alleged Victim (AV) is known to exit seek. AV was transferred from an unlocked facility due to wandering and exit seeking. Facility staff were called on or about April 20, 2024 and advised that AV was outside of the building. AV had opened a window and kicked out the screen and exited the building through a window and was seen by staff at a neighboring building. Facility staff retrieved AV and brought him/her back to the building without injury, however, AV was placed at risk for harm. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
