Findings: The Alleged Victim relies on the facility for his or her safety and has a history of exit seeking. On or about April 30, 2024, the facility conducted a fire drill and failed to watch one of the secured doors that are automatically unlocked when the fire alarm goes off. AV was observed outside of the building in the parking lot a short time later, and staff were able to coax AV back inside the building. The facility failed to provide a safe environment, which is a violation of resident’s rights, is considered neglect of care and constitutes abuse.
