Findings: According to the documentation, the facility failed to follow the care plan for the Alleged Victim’s (AV) known fall risk. On or about May 04, 2024, at approximately 1:15 am, the AV was found on the ground next to their bed. The facility staff put the AV back in bed. Later that same morning at approximately 6:54 am, the AV was again found on the fall mat next to their bed complaining of back pain. There was no documented evidence the facility followed the care plan for safety checks and placed the tab alarm to notify staff if the AV was trying to get out of bed, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
