Findings: According to the documentation, the facility failed to provide a safe environment by leaving the backyard gate unlocked. On or about May 01, 2024, the Alleged Victim (AV) moved into the facility and became agitated when their family left. The AV then entered the secured back yard. The gate in the back yard was not locked, allowing the AV to leave the facility unaccompanied by a staff member. Outside the gate is a busy roadway which put the AV at risk of harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP24-01215 $338.00 fine assessed
