Findings: According to the documentation, the facility failed to provide a safe environment for the Alleged Victim (AV) to ensure they did not leave the secured facility. On or about May 08, 2024, at approximately 2:30 in the afternoon, the AV was in the hallway outside of the secured facility. The Av was seen approximately 10 minutes prior to being found outside the secured doors of the facility, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
