Findings: The Alleged Victim (AV) relies on the facility to for care and supervision. The AV is a known fall risk. According to an investigation, on or about May 10, 2024, AV was found by facility staff sitting awake on the edge of the bed with injuries to AV’s face, head and hand. The facility failed to provide appropriate services according to Alleged Victim’s needs, relating to care planning and lack of appropriate interventions, to mitigate the risk of injury due to AV's increase in falls, which is a violation of resident rights, is neglect of care which constitutes abuse.
Sanction: RCFCP25-00743 $375.00 fine assessed
