Findings: The Alleged Victim (AV) and Witness #1 (W1) have a history of alterations. On March 3, 2024 AV and W1 were involved in an altercation where W1 suffered a skin tear. No interventions were put into place after this altercation. On or about May 2, 2024, W1 pushed AV into the door frame of his/her room. AV was calling for help while W1 was yelling at AV and hitting AV in the back. AV tried to get away, W1 grabbed AV's hand, causing a skin tear and a large bruise. The facility's failure to ensure the safety of residents is a violation of resident rights, is considered neglect of care and constitutes abuse.
