Findings: The Alleged Victim (AV) and Witness #1 (W1) have been involved in multiple altercations. On or about May 6, 2024, AV and W1 engaged in an altercation where W1 grabbed onto AV's arm, causing a skin tear. The facility failed to properly care plan to reduce the risk of altercations, placing residents at risk of harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
